Hypoparathyroidism-related Fahr syndrome presenting with new-onset seizures: a case report

Background:
Hypocalcemia secondary to hypoparathyroidism can cause basal ganglia calcification which can act as epileptogenic focus and produce neuropsychiatric manifestations. This is termed Fahr syndrome.
Case Presentation:
A 36-year-old Nepalese female with hypoparathyroidism and history of anxiety disorder presented with generalized seizures. She was found to have bilateral calcification in brain consistent with Fahr syndrome. She was successfully managed with calcium and calcitriol supplementation.
Conclusion:
This case highlights the need for recognizing basal ganglia calcification as an additional cause of seizures in hypoparathyroidism, in addition to hypocalcemia.
