Understanding Kratom: Pharmacology, Risks, and Relevance to South Dakota Providers

Introduction:
Kratom (Mitragyna speciosa), a Southeast Asian botanical, has gained increasing popularity in the U.S. for its stimulant and opioid-like effects. As its use grows, providers face challenges in understanding its pharmacology, clinical risks, and regional impact.
Methods:
This narrative review synthesizes existing literature on kratom's pharmacological profile, clinical implications, regulatory status, and poison control trends, with a particular emphasis on South Dakota.
Results:
Kratom contains mitragynine and 7-hydroxymitragynine, alkaloids with activity at opioid receptors. Its effects vary by dose, ranging from stimulation to sedation. Though often used for pain and opioid withdrawal, kratom carries risks such as dependency, hepatotoxicity, seizures, and death-particularly when combined with other substances. South Dakota-specific poison control data reveal a significant uptick in kratom-related calls, aligning with national increases.
Conclusion:
Kratom remains legal and accessible in South Dakota, contributing to rising clinical encounters. Providers should screen for use, understand its dose-response profile, and be prepared to address associated complications.
