The fire that reveals: A physician's reflection on burnout, meaning, and the work of healing

Abstract:
Burnout is conventionally described from the outside: emotional exhaustion, depersonalization, and reduced professional accomplishment, measured by validated instruments and framed as an occupational phenomenon. This reflection describes it from the inside. Drawing on the author's experience as a physician who developed fibromyalgia at age thirty-nine and major depression at forty after years of unexamined overwork, the essay explores what the standard framework of burnout captures well and what it quietly leaves out: the body that testifies before the mind admits anything; the erosion of meaning that precedes the erosion of energy; the difference between compassion fatigue and the deeper spiritual weariness that ancient contemplative traditions called acedia; and the counterintuitive lesson, drawn from both palliative care and the biblical narrative of Elijah, that restoration begins with bread and sleep, not with exhortations about purpose. The essay argues that healing from burnout - as distinct from merely recovering from it - requires attending to four dimensions of the person at once: biological, psychological, social, and spiritual. It closes with a question for clinicians and healers of every tradition: not how to extinguish the fire at any cost, but how to let what burns reveal what must be rebuilt.
