Intraoral Excision of an Anteromedial Mandibular Condylar Osteoma Combined With Simultaneous Orthognathic Correction

Abstract:
Mandibular condylar osteoma is a rare benign tumor that may cause progressive facial asymmetry, impaired mastication, and temporomandibular joint dysfunction. Conventional tumor removal through a lateral preauricular extraoral approach is particularly challenging for lesions arising from the anteromedial condyle because of limited access, risk of facial nerve injury, and external scarring. In patients with severe secondary dentofacial deformities, tumor excision alone may also fail to restore functional occlusion. We report a 30-year-old patient with progressive mandibular deviation, malocclusion, and a large osseous mass arising from the anteromedial condyle. An intraoral approach combined with bilateral sagittal split ramus osteotomy was performed, allowing safe tumor removal and simultaneous correction of the mandibular deformity. Postoperatively, facial symmetry and occlusal function were restored without facial nerve deficit or visible scarring. This approach may be a feasible and effective scar-avoiding option for selected condylar osteomas with dentofacial deformity.
