Abstract:
A man in his 60s with type 2 diabetes mellitus and hypertension presented with complaints of constant brown spots in his vision 2 weeks after he experienced cardiac arrest during an endoscopy with general anaesthesia. Dilated fundus examination (DFE) showed radial reddish-brown lesions around the fovea bilaterally, which were also seen on fundus photography. Areas of hyper-reflectivity between the outer retinal layers and disruption of the ellipsoid zone were noted on spectral domain-optical coherence tomography of both eyes. One month after presentation, the patient reported resolution of the scotomas and the lesions appeared to be improving on DFE. This is the first reported case of acute macular neuroretinopathy after either endoscopy or general anaesthesia associated with cardiac arrest.
