Deep endometriosis affecting the lateral pelvic structures (LPS) is a complex and often underdiagnosed manifestation of the disease. The authors review the anatomic features of the LPS and underscore the importance of MRI in detecting lesions and assessing neurovascular involvement. A solid understanding of this anatomy is key to accurate diagnosis, surgical planning, and avoiding misdiagnosis. The authors advocate a multidisciplinary approach and highlight MRI as an essential tool for preoperative staging and treatment planning in patients with suspected LPS involvement. ©RSNA, 2026 Supplemental material is available for this article.
