Role of Magnetic Resonance Elastography in the Assessment of Liver Fibrosis: A Descriptive Study

Context:
Liver fibrosis is a key consequence of chronic liver diseases (CLDs), leading to cirrhosis and hepatocellular carcinoma. Early detection and accurate staging are crucial for management. Magnetic resonance elastography (MRE) offers a noninvasive modality for assessing liver stiffness.
Aims:
The aim of the study was to evaluate the extent and severity of liver fibrosis in patients with CLD using MRE and to correlate MRE findings with ultrasonography (USG) and clinical parameters.
Settings And Design:
An observational, descriptive study.
Subjects And Methods:
The study was conducted on 45 patients with USG-diagnosed CLD. All participants underwent MRE using a 3T Siemens MAGNETOM Vida magnetic resonance imaging (MRI) scanner. Liver stiffness measurements were recorded and staged using Meta-analysis of Histological Data in Viral Hepatitis (METAVIR scoring system for grading activity and staging fibrosis in chronic hepatitis) equivalents. Clinical, laboratory, and USG data were collected and analyzed. Of 45 patients, 62.2% were male, and 48.9% were obese. Alcoholic liver disease (44.4%) and nonalcoholic fatty liver disease (40%) were predominant etiologies. Most patients (66.7%) had mild fibrosis (stage 1). Mean liver stiffness was 4.84 ± 2.30 kPa. USG echotexture correlated significantly with MRI fat fraction (P = 0.003).
Conclusion:
This study highlights the pivotal role of MRE in the comprehensive evaluation of liver fibrosis in patients with CLD. Our findings underscore MRE's ability to detect and stage fibrosis with high sensitivity, even in the early stages of disease when ultrasound or laboratory tests might be inconclusive. The strong correlation between MRE-based stiffness measurements and clinical or sonographic features reinforces its diagnostic accuracy. By offering a noninvasive, reproducible, and operator-independent assessment, MRE addresses the limitations of traditional liver biopsy and ultrasound-based techniques and can significantly influence treatment decisions and patient outcomes.
