Interstitial cystitis with Hunner's ulcers managed with Jatyadi Taila Uttarabasti - A case series

Abstract:
Interstitial cystitis (IC) with Hunner's ulcers is a chronic inflammatory condition often resistant to conventional therapies, which frequently offer only temporary relief or carry risks of bladder scarring. This report describes the clinical outcomes of Jatyadi Taila Uttarabasti (intravesical instillation) combined with Virechana Karma (purgation) and oral formulations-in managing severe IC. Two patients with chronic IC and Hunner's ulcers, non-responsive to conventional treatments, received individualized Ayurvedic treatment at the hospital. Clinical progress was documented using the Visual Analogue Scale (VAS) for pain, the King's Health Questionnaire (KHQ), and the IC-Quality of Life (IC-QoL) score, with a follow-up period of two years. Both patients demonstrated significant clinical improvement. Pain scores reduced from severe to near-zero, and urinary frequency and nocturia were normalized. Quality-of-life metrics showed substantial enhancement, with IC-QoL scores reflecting near-complete symptom resolution. Notably, these results remained stable throughout the two-year follow-up period. Patient 2 opted for a planned maintenance course in late 2024 to ensure clinical stability, despite the resolution of primary symptoms. This case series suggests that Ayurvedic management, particularly the use of Jatyadi Taila Uttarabasti for its wound-healing and anti-inflammatory properties, provides a cost-effective, non-invasive, and long-lasting alternative for severe IC and Hunner's ulcers.
