•Depression and apathy can confound post-stroke cognitive test performance.•Validated post-stroke depression and apathy scales should be used routinely.•Relying only on MoCA may miss focal and domain-specific post-stroke deficits.•Stroke-specific screens (e.g. OCS, ACE-III, CCAS-S) complement global measures.•Tailored screening plus neuropsychiatric assessment improves PSCI evaluation.
