The Readiness Paradox: Beyond Financial Transparency in the Military Health System

This commentary explores the strategic implications of the Department of Defense's (DoD) proposed FY2027 budget separation of direct care funding from Private Sector Care Programs (PSCP). While this reform improves financial transparency within the Military Health System (MHS), transparency alone does not preserve the behavioral incentives necessary to sustain the clinical workload required for operational medical readiness. We introduce the "Readiness Paradox"-the unique mandate for Military Treatment Facilities (MTFs) to maintain readiness-generating capabilities regardless of fluctuations in patient demand. Using pharmacy utilization and a recent MTF reattraction pilot as a strategic case study, we illustrate how targeted interventions can influence beneficiary behavior, recapture clinical workload, and moderate purchased care growth. Ultimately, we argue that future MHS reforms must move beyond accounting clarity to establish robust incentive structures that prioritize beneficiary retention and preserve the clinical volume necessary to sustain military medical readiness.
