Early T cell-mediated rejection (TCMR) in kidney transplants is common and often resistant to treatment. Persistent TCMR significantly increases the risk of graft loss, challenging the view that it is benign.

- Findings necessitate a re-evaluation of TCMR's clinical significance and underscore the need for targeted therapies.
