Vertical phakic intraocular lens implantation yields superior astigmatic stability compared to horizontal

Objective:
This study aimed to evaluate the effect of the implantation meridian (horizontal or vertical) of the Eyecryl phakic intraocular lens (pIOL) on ocular residual astigmatism (ORA), anterior chamber depth (ACD), and vault height.
Methods:
A total of 44 eyes were retrospectively analyzed and divided into two groups based on the meridian of pIOL implantation: horizontal (n = 16) and vertical (n = 28). Keratometry, refractive astigmatism, ORA, ACD, and vault height were compared between the groups.
Results:
A significant postoperative increase in ORA was observed in the horizontal group (P < 0.001), whereas ORA remained stable in the vertical group. ACD reduction was more pronounced in the horizontal group (P = 0.006). No significant difference was found between the groups in terms of changes in vault height (P > 0.05).
Conclusion:
In this study, ORA values remained stable in the group with vertically implanted pIOLs during both the preoperative and postoperative periods. This finding suggests that vertical implantation provides more stable refractive outcomes.
