With improved prognosis in early HER2-positive breast cancer, safely de-escalating treatment is increasingly investigated to minimize overtreatment. This article focuses on neoadjuvant de-escalation strategies for HER2-positive breast cancer. There have been initiatives to shorten the duration of anti-HER2 therapy or to reduce the use of chemotherapy agents. Predictive or prognostic biomarkers and imaging techniques might help to identify subsets of patients within HER2-positive cancer with the best chance of responding to treatment and benefiting from de-escalated treatment.
