Transcatheter Closure of Aortic Paravalvular Leak After the Ozaki Procedure

Background:
Aortic valve disease requires individualized treatment strategies. The Ozaki procedure is an alternative to valve replacement; however, paravalvular leak (PVL) remains a potential complication.
Case Summary:
A 62-year-old man with previous Ozaki procedure presented with NYHA functional class III dyspnea. Echocardiography revealed moderate-to-severe PVL between the right and noncoronary cusps. Because of high surgical risk (Society of Thoracic Surgeons score 9.1%), transcatheter closure was performed under transesophageal echocardiography guidance using a 5 × 4-mm Amplatzer Duct Occluder II via transfemoral access. The device was successfully deployed with trivial residual regurgitation. The patient was discharged in stable condition with clinical improvement at 6 months.
Discussion:
This case demonstrates the feasibility and safety of transcatheter PVL closure after the Ozaki procedure, particularly in high-risk patients, highlighting its potential as a minimally invasive alternative.

Transcatheter PVL closure appears to be a safe and effective treatment choice after the Ozaki procedure. Careful imaging guidance is essential for optimal outcomes.
