Thrombosis can occur even with direct oral anticoagulants (DOACs) in atrial fibrillation (AF). A patient with AF and a Watchman device developed deep vein thrombosis and left atrial thrombus, revealing a hypercoagulable state. Further investigation is crucial when anticoagulation fails.
