Outcomes of oculomotor nerve palsy treated with augmented transposition: a case series

Abstract:
We present a series of 3 patients with partial oculomotor nerve palsy who underwent a modified augmented transposition procedure, reported here for the first time, which effectively improved both horizontal and vertical deviations. This procedure consists of resections of both superior rectus and medial rectus muscles, followed by transpositions of the medial rectus muscle superiorly and the superior rectus muscle nasally to join in the superonasal quadrant. This technique avoids operation on three or more extraocular muscles, which is typically required to achieve orthotropia in primary position for oculomotor nerve palsies, consequently reducing the risk of anterior segment ischemia postoperatively.
