Administering Blinatumomab via Continuous Intravenous Infusion in Pediatric Patients-A Single-Institution

Blinatumomab, a CD19xCD3 bispecific T-cell engager, has become standard therapy for children with de novo B-cell acute lymphoblastic leukemia (B-ALL). This retrospective review describes the experience of blinatumomab administration in pediatric patients. At our institution, 31 children with B-ALL received continuous intravenous (IV) blinatumomab from January 1, 2018, to December 31, 2024. Logistical issues requiring clinical intervention occurred in 16 (51.6%) patients, with 57.8% of these issues resulting in hospitalization. After receiving blinatumomab, 26 patients (83.9%) required intravenous immunoglobulin (IVIG). These results support investigations into alternative methods of blinatumomab delivery and the impact of blinatumomab on immune function in pediatric patients with B-ALL.
