In patients with moderate-to-severe COPD, short-term (1-day) preoperative respiratory optimization may be insufficient to prevent acute exacerbation after extubation. Prolonged preoperative preparation, meticulous intraoperative lung-protective ventilation, and a post-extubation rapid-response protocol are essential.
