Purely intraventricular craniopharyngiomas are uncommon, and their complete removal through a neuroendoscopic transventricular approach has only occasionally been described. We report the case of a 38 years-old-female presenting with headache and papilledema. Brain MRI revealed a mass located in the left foramen of Monro extending into the third ventricle, without suprasellar involvement. A neuroendoscopic transventricular approach was used for excision. Histopathology confirmed papillary craniopharyngioma. The patient recovered without significant neurological deficits. This case supports the feasibility of complete endoscopic removal of selected intraventricular craniopharyngiomas.
