MAG3 SPECT/CT Detection of Vesicocolic Fistula in Genitourinary Tuberculosis

Enterovesical fistula is a rare complication of genitourinary tuberculosis and may be difficult to diagnose, particularly in the setting of impaired renal function. We report a 41-year-old woman with chronic kidney disease and bilateral obstructive uropathy secondary to treated genitourinary tuberculosis. 99mTc MAG3 renography demonstrated a nonfunctioning left kidney and preserved right renal function without obstruction. Delayed images revealed abnormal tracer propagation from the urinary bladder into bowel loops. SPECT/CT localized the tracer within the colon, confirming a vesicocolic fistula.
