Taurodontism as a predisposing factor for cracked tooth syndrome

A tooth with taurodontism is characterized by the absence of constriction at the level of the cementoenamel junction and is distinguished by abnormally large pulp chambers. In the present case, the pulpal floor is displaced more apically as a result of the expanded pulp chamber, and bi/trifurcation of the roots occurs at that level. Due to its structural and anatomical variations, this developmental anomaly can alter the biomechanics of the tooth. Additional stresses on such a tooth can result in crack formation and propagation leading to cracked tooth syndrome (CTS). Depending on the extent of the crack and any potential microbial infections, cracked teeth can cause a range of clinical symptoms. Management of CTS varies based on the involved tooth, extent of the crack, and severity of symptoms. Since the prognosis of such a tooth is still uncertain, continuous assessment is essential. Thus, endodontic therapy in a taurodontic cracked tooth is often challenging for practitioners. This article discusses an interesting case report on the management of a mesotaurodontic tooth associated with crack tooth syndrome.
