Minimally Invasive Versus Conventional Interval Cytoreduction Surgery for Epithelial Ovarian Carcinoma: A Randomized

Abstract:
Optimal cytoreduction improves survival in advanced epithelial ovarian cancer. Minimally invasive surgery (MIS) may offer perioperative advantages over open surgery, but evidence is limited. This study compares MIS versus open interval cytoreduction following neoadjuvant chemotherapy. In this prospective, randomized controlled trial, 40 women with FIGO stage III-IV epithelial ovarian carcinoma responding to neoadjuvant chemotherapy were randomized to undergo either MIS or open surgery at Mansoura University Oncology Center from July 2021 to December 2024. Primary outcomes included disease-free survival (DFS) and overall survival (OS). Secondary outcomes included operative time, blood loss, complications, and hospital stay. Follow-up was at least 24 months. Baseline characteristics were comparable. MIS resulted in significantly shorter operative time, less blood loss, and shorter hospital stay. No significant differences were observed in intraoperative complications, DFS, or OS between groups at follow-up. Recurrence rates were similar. MIS can be considered a safe, feasible technique for selected patients with advanced ovarian cancer responding to neoadjuvant chemotherapy, offering perioperative benefits without compromising oncologic outcomes.
