Median Craniofacial Dysplasia: Operative Decision-Making Guided by Tissue Substrate

Background:
Median craniofacial dysplasia comprises a rare and heterogeneous group of midline deformities. Although embryologic and anatomic classifications describe the morphologic spectrum, practical guidance for operative decision-making remains limited. Patterns of secondary deformity observed in referral practice suggest that operative strategy may not always align with the underlying tissue substrate.
Methods:
A descriptive retrospective observational case series spanning the spectrum of median craniofacial dysplasia was reviewed to develop a hypothesis-generating conceptual operative framework derived from longitudinal institutional experience. Deformities were categorized as deficiency (hypoplasia), abnormal separation with preserved volume (dysraphia), or excess (hyperplasia). Principles of operative intent, timing, and staging were derived from longitudinal clinical observations.
Results:
Distinct reconstructive patterns were observed across tissue substrates within this heterogeneous retrospective cohort. Among hypoplastic patients, secondary referrals following prior intervention frequently demonstrated progressive nasal narrowing and functional compromise. These observations should be interpreted descriptively and may reflect intrinsic substrate deficiency, referral bias, previous treatment, or a combination of these factors. Dysraphic deformities generally maintained stable continuity following early reapproximation. Hyperplastic deformities more commonly required staged reduction and delayed skeletal correction during growth.
Conclusions:
Median craniofacial dysplasia may be more usefully approached through tissue substrate in addition to cleft anatomy alone. This substrate-based framework is intended as a hypothesis-generating conceptual framework for operative reasoning in deformities where anatomy alone may not adequately guide timing or staging.
