This case presents a patient in her early 30s who had a heterotopic caesarean scar pregnancy following spontaneous conception. This is a rare presentation, so the literature is limited on diagnosis, management and outcomes. In this case, both the caesarean scar heterotopic and the intrauterine pregnancies were alive. The patient successfully underwent surgical management of heterotopic pregnancy. Given the rarity of this presentation, a detailed description of the management is important. Due to the global increase in caesarean section rates and assisted reproductive technologies, more research is required to establish guidelines on investigation and management of heterotopic caesarean scar pregnancy.
