Adalimumab significantly reduced treatment burden for idiopathic multifocal choroiditis/punctate inner choroidopathy. This biologic therapy decreased steroid use, injections, and other immunomodulatory therapies while preserving vision.
Area of Science:
- Ophthalmology
- Immunology
- Retinal Diseases
Background:
- Idiopathic multifocal choroiditis (MFC) and punctate inner choroidopathy (PIC) are inflammatory retinal conditions.
- Current treatments often involve significant systemic or local immunosuppression, leading to treatment burden.
Purpose of the Study:
- To evaluate the efficacy of adalimumab (ADA) in reducing the overall treatment burden in patients with idiopathic MFC/PIC.
- To assess the impact of ADA on medication dosage, procedures, and disease flares.
Main Methods:
- Retrospective case series of patients with idiopathic MFC/PIC.
- Data collected included best-corrected visual acuity (BCVA), corticosteroid dosage, immunomodulatory therapy (IMT) use, anti-vascular endothelial growth factor (anti-VEGF) injections, and disease flares.
- Evaluations were performed at baseline, 12 months, and last follow-up.
Main Results:
- Twenty-five patients (41 eyes) were included with a mean follow-up of 51.4 months.
- Adalimumab therapy led to a significant reduction in prednisone dosage (15.76 mg/day to 0.23 mg/day, p<0.0001).
- There was a significant decrease in steroid/anti-VEGF injections (30 to 1, p<0.0001) and other IMT use (8 to 1, p<0.05).
Conclusions:
- Adalimumab demonstrated a significant steroid-sparing effect in patients with idiopathic MFC/PIC.
- The therapy helped preserve vision and reduce the need for other intensive treatments.
- Adalimumab effectively lowered the overall treatment burden associated with these conditions.
