The United States has an exceptionally high preterm birth rate, which features inequities that impact many Black, Indigenous, and Pacific Islander families. Unconditional perinatal cash transfers, which offer cash assistance to pregnant and postpartum women with no strings attached, are a promising tool to address disparate birth outcomes. Canada and the United States both have an emerging evidence base that supports the impact of perinatal cash transfers on birth outcomes for all groups, but especially for those who are from under-resourced communities. We review the evidence supporting the use of perinatal cash transfers in Canada and the United States, discuss the distinct advantages of perinatal cash transfers, and highlight examples of growing momentum and support for this type of intervention. Although there are still challenges to their widespread expansion and adoption, we argue that perinatal cash transfers should be included as an essential tool in the US public health toolbox to advance health and financial equity while optimizing dignity, autonomy, and flexibility for families in a changing policy landscape.

Development of the Oral Microbiota01:28

Development of the Oral Microbiota

The establishment of the oral microbiome begins before birth, challenging the long-held belief that the fetal oral cavity is sterile. The presence of oral microbes such as Streptococcus and Fusobacterium in amniotic fluid suggests that microbial exposure may occur in utero, potentially through translocation from the maternal oral or gastrointestinal tract. This early colonization primes the neonatal immune system and sets the stage for subsequent microbial succession. Maternal health,...

Methods Of Healthcare Delivery System

At the different levels of the healthcare system, we see varying methods of healthcare used. These methods include managed care systems, case management, and primary healthcare.
Managed Care System:
The managed care system is designed to control the cost while maintaining the quality of care. The patient's care from admission to discharge is planned by the primary care provider or the case manager, also known as the gatekeeper. In a managed care system, the number of care providers is limited...
Transcytosis of IgG01:15

Transcytosis of IgG

Transcytosis is the process in which molecules are internalized by endocytosis, transported across the cell, and released through exocytosis from the opposite end of the cell. Molecules such as insulin, immunoglobulins, and certain nutrients are transferred through the recycling endosomes by recycling and transcytosis.
IgG molecules from a mother undergo transcytosis starting around 13 weeks of gestation. The amount of IgG transferred and entering the fetal blood circulation increases with...
