Background And Objective:
Ultra-high-field 7-Tesla (7-T) magnetic resonance imaging (MRI) has emerged as an advanced neuroimaging modality that offers substantially higher signal-to-noise ratio (SNR), improved contrast resolution, and enhanced metabolic characterization compared with conventional 1.5- and 3-T MRI. These technical advantages have generated increasing interest in neuro-oncology, where accurate tumor detection, characterization, treatment planning, and response assessment are critical. This review aims to summarize current clinical and translational applications of 7-T MRI in neuro-oncology, evaluate its advantages and limitations, and discuss future directions for its integration into clinical practice.
Methods:
A structured narrative review was conducted using PubMed, Scopus, and Web of Science databases. Relevant studies published up to November 2025 were identified using combinations of keywords related to 7-T MRI, UHF imaging, neuro-oncology, gliomas, pituitary adenomas, sellar, skull-base tumors, advanced MRI techniques, and metabolic imaging. English-language original studies, prospective cohorts, systematic reviews, and technically significant proof-of-concept investigations were prioritized. Reference lists of eligible studies were additionally screened to identify relevant publications.
Key Content And Findings:
The review discusses the technical foundations of 7-T MRI and its current applications in tumor diagnosis, surgical planning, radiotherapy planning, and therapy monitoring. Evidence indicates that 7-T MRI improves visualization of tumor microvasculature, infiltrative margins, susceptibility-related abnormalities, and treatment-induced microvascular injury. Advanced techniques, including susceptibility-weighted imaging (SWI), high-resolution diffusion imaging, magnetic resonance spectroscopy (MRS), sodium MRI, phosphorus spectroscopy, and chemical exchange saturation transfer (CEST) imaging, provide enhanced metabolic and molecular characterization of brain tumors. Promising applications have been reported for gliomas, pituitary adenomas, meningiomas, metastatic disease, and selected pediatric tumors. However, most available evidence demonstrates technical and diagnostic superiority, whereas robust evidence for improved clinical outcomes remains limited.
Conclusions:
7-T MRI offers significant potential to advance precision neuro-oncology through superior structural, functional, and metabolic imaging. Nevertheless, widespread clinical adoption remains constrained by technical challenges, cost, limited availability, and lack of standardized protocols. Future multicenter prospective studies, protocol harmonization, and outcome-based validation are needed to establish the clinical impact of 7-T MRI and guide future research, clinical implementation, and healthcare policy development.
