SUSTAIN-RO study: a nationwide pilot survey on environmental sustainability practices among Spanish radiation

Purpose:
To characterize sustainability practices and identify implementation barriers among radiation oncology (RO) professionals in Spain.
Material And Methods:
A 38-item survey was distributed to members of the Spanish Society of Radiation Oncology (SEOR) in October 2025. Responses from 122 specialists (out of 1355 invited, 9.0% response rate) were analyzed using Chi-Square and Kruskal-Wallis tests.
Results:
Private vehicles remain the primary mode of commuting (56.2%), even though 61.5% of specialists live within 10 km of their centers. Significant barriers include a 75.4% lack of formal sustainability training and a 92% absence of teleworking options. However, the public sector shows a significantly lower adoption of paperless workflows compared to the private sector (29.1% vs. 69.2%; p = 0.016). Interest in national green initiatives is high (median 8.0/10).
Conclusion:
A significant gap exists between professional interest and institutional support. Education and improving departmental infrastructure are key to advancing sustainable RO in Spain.
