Background:

  • Amyloid-β (Aβ) Positron Emission Tomography (PET) imaging is known to bind to white matter myelin.
  • The influence of intracortical myelin on cortical Aβ PET quantification is not well understood.

Purpose of the Study:

  • To investigate if myelin water fraction (MWF), an MRI measure of myelin, improves the correlation between Aβ PET and plasma p-tau217 in Alzheimer's Disease (AD).

Main Methods:

  • 114 participants with/without AD underwent Aβ PET (11C-Pittsburgh compound B or 18F-florbetaben), multiecho fast T2 MRI for MWF quantification, and plasma p-tau217 measurement.
  • Nested regression models predicted PET signal using age and p-tau217, with and without cortical MWF.

Main Results:

  • Myelin water fraction (MWF) independently predicted PET-measured Aβ signal.
  • Incorporating MWF improved the model fit for Aβ PET quantification by approximately 6%.

Conclusions:

  • Intracortical myelin significantly contributes to the Aβ PET signal.
  • Accounting for MWF may enhance Aβ PET quantification accuracy for AD diagnosis, patient selection, and therapy monitoring.
