Abstract:
Arteriovenous graft wall delamination is a rare complication that can compromise vascular patency. Definitive consensus on optimal management strategies for this condition is lacking, and existing strategies are largely empirical. We report the case of a 55-year-old man with end-stage renal disease who presented with upper-extremity early-cannulated arteriovenous graft thrombosis 6 months after implantation. Color Doppler ultrasound study revealed graft wall delamination and false lumen formation. The patient underwent thrombectomy, ultrasound-guided needle aspiration for false lumen volume reduction, and angioplasty. Unobstructed graft function was maintained at the 6-month follow-up. Improper cannulation may cause this structural injury; endovascular angioplasty combined with false lumen reduction yields favorable midterm efficacy.
