Comprehensive exercise countermeasures for astronauts low back pain in microgravity: A scoping review

Objective:
To systematically review the current research status and effectiveness of comprehensive exercise countermeasures for astronauts low back pain (LBP) in microgravity.
Methods:
A comprehensive literature search was conducted in PubMed and Web of Science for relevant studies published between October 2015 and October 2025, focusing on exercise interventions for LBP in microgravity. A scoping review methodology was employed to map the available evidence.
Results:
A total of 10 studies from 6 countries, involving 152 participants, were included. The study designs comprised randomized controlled trials, prospective cohort studies, cross-sectional studies, and case analyses. The primary exercise countermeasures included core and stability training, traditional and advanced resistance training, as well as dynamic and aerobic exercises. The evidence indicates that comprehensive exercise countermeasures can effectively alleviate LBP, improve spinal stability, and partially reverse intervertebral disc expansion and muscle atrophy.
Conclusion:
Comprehensive exercise countermeasures exert positive effects and show promising application prospects in the prevention and treatment of astronauts' low back pain under microgravity conditions.
