Comparative Evaluation of Esthetic Outcomes Among Different Dental Implant Systems in the Maxillary Anterior Region

Abstract:
Introduction Achieving optimal esthetic outcomes in implant-supported restorations in the maxillary anterior region remains a major challenge in implant dentistry. Implant design, implant-abutment connection, prosthetic rehabilitation, and peri-implant soft tissue stability may significantly influence the final esthetic outcomes. A comparative evaluation of different implant systems using standardized esthetic indices may assist clinicians in selecting appropriate implant systems for esthetically demanding cases. Therefore, this study aimed to evaluate and compare the esthetic outcomes of Straumann, Nobel Biocare, and Osstem implant systems in the maxillary anterior region. Materials and methods This prospective observational study was conducted in the Department of Prosthodontics, Crown and Bridge, over a period of two years. A total of 90 patients receiving single implant-supported restorations in the maxillary anterior region were enrolled and prospectively followed. Based on the implant system used, participants were categorized into three groups: Straumann, Nobel Biocare, and Osstem (n = 30 each). Esthetic outcomes were evaluated clinically using the Pink Esthetic Score (PES), White Esthetic Score (WES), and Visual Analog Scale (VAS) six months after definitive prosthetic loading. Statistical analysis was performed using one-way analysis of variance (ANOVA) for continuous variables and the chi-square test for categorical variables. A p-value < 0.05 was considered statistically significant. Results Among the study participants, 43 (47.8%) belonged to the 36-50 years age group, 56 (62.2%) were females, and 70 (77.8%) were non-smokers. Straumann implants demonstrated the highest mean PES score (12.1 ± 1.4), followed by Nobel Biocare (11.8 ± 1.5) and Osstem implants (9.9 ± 1.7), with statistically significant differences among the groups (p < 0.001). Similarly, the mean WES scores were significantly higher for Straumann (8.5 ± 0.9) and Nobel Biocare implants (8.3 ± 1.0) than for Osstem implants (7.1 ± 1.2) (p < 0.001). The mean VAS score was also the highest for Straumann implants (88.4 ± 7.2), followed by Nobel Biocare (85.6 ± 8.1) and Osstem implants (74.3 ± 10.5) (p < 0.001). Straumann and Nobel Biocare implants exhibited significantly superior esthetic outcomes than Osstem implants. Conclusion Within the limitations of the present study, Straumann and Nobel Biocare implant systems demonstrated superior esthetic outcomes compared with Osstem implants in the maxillary anterior region. The implant-abutment connection design, use of custom abutments, and prosthetic characteristics may significantly influence peri-implant esthetics and overall patient satisfaction.
