Primary hyperoxaluria type 1 is a rare genetic disorder that leads to systemic oxalate deposition and end-stage renal disease. We describe 68Ga-FAPI-04 PET/CT findings in a case of primary hyperoxaluria type 1. 68Ga-FAPI-04 PET/CT showed striking FAPI uptake in multiple bones and joints. This FAPI distribution likely represents a unique imaging manifestation of the active fibroblastic repair process triggered by oxalate crystal deposition.
