Despite standardized patient selection, the specific technical execution of genicular artery embolization (GAE) is not yet uniform, suggesting a need for further investigation into how these variations might influence long-term results. This variability reflects procedural factors including incomplete neovessel detection, inconsistent assessment of genicular anatomic variants, and heterogeneous embolization strategies. This review proposes a systematic technical framework designed to standardize GAE performance and improve reproducibility. We outline a structured approach to vascular access, stepwise neovessel detection using optimized angiographic projections and cone-beam CT, anatomic decision-making in the presence of common variants and anastomoses, and controlled free-flow embolic delivery strategies tailored to vessel morphology and risk profile. Postprocedural management and structured follow-up protocols are discussed to align patient expectations with outcome assessment. By integrating imaging precision with a disciplined embolization technique, this framework aims to reduce operator-dependent variability and support consistent clinical outcomes.
