Using data from 4549 US adults in a national cohort and a difference-in-differences design, we estimated the average treatment effect on the treated to assess whether receiving tests from COVIDTests.gov-a free, direct-to-household program launched in January 2022-increased testing for severe acute respiratory syndrome coronavirus 2. Receiving tests was associated with a 13.8-percentage-point testing increase in March 2022 (95% confidence interval = 3.3, 24.2), the only estimate robust to pretrend sensitivity adjustments, with later effects attenuating. Participation was lower among groups with greater health risks and access barriers. (Am J Public Health.
