Nonspecific Effects in Atrial Fibrillation Ablation: Insights From SHAM-PVI and Other Sham-Controlled Trials,

Abstract:
Sham-controlled procedural trials demonstrate that objective clinical endpoints may improve substantially independent of the index therapeutic mechanism. The authors evaluated sham-controlled cardiovascular trials with subjective and objective endpoints to estimate nonspecific procedural effects and applied this framework to atrial fibrillation (AF) ablation. The nonspecific fraction (ƒ), defined as the proportion of within-arm change observed in the sham arm relative to the active arm, ranged from 0.09 to 0.71 across renal denervation and percutaneous coronary intervention trials. In SHAM-PVI, continuous implantable loop recorder monitoring demonstrated a 35.0 percentage-point reduction in AF burden in the sham arm (ƒ≈0.58). Sensitivity modeling estimated lesion-specific AF-burden reduction as (1-ƒ) × observed reduction, suggesting that 25% to 70% of within-ablation AF-burden reduction may reflect nonspecific procedural effects. These findings provide a practical framework for interpreting AF-burden endpoints and emphasize the importance of sham-controlled trial design in distinguishing lesion-specific therapeutic benefit from nonspecific procedural influences.
