How Disease Progression Changes Operative Thresholds in MEN1: A Case-Based Management Review

BackgroundMultiple Endocrine Neoplasia type 1 (MEN1) is a rare autosomal dominant disorder characterized by primary hyperparathyroidism, pancreatic neuroendocrine tumors (pNETs), and pituitary tumors. Management is complicated by the asynchronous, multifocal presentation of endocrine lesions over years, where each intervention alters the risks, options, and timing of future treatment. The central challenge in advanced MEN1 is how to sequence intervention across multiple competing endocrine and oncologic priorities while preserving long-term pancreatic function and quality of life.Case ReportWe present a case of a 35-year-old female with MEN1 illustrating how operative thresholds evolve over a decade of longitudinal care. Initial presentation with a vasoactive intestinal peptide (VIP)-producing tumor (VIPoma) required distal pancreatectomy and splenectomy. Subsequent development of primary hyperparathyroidism, papillary thyroid cancer, and pituitary adenoma were managed with parathyroidectomy and thyroidectomy, while pancreatic gastrinoma was initially observed to preserve residual pancreatic endocrine function. The appearance of hepatic metastases altered the operative threshold, prompting completion pancreatectomy with liver resection despite the known consequence of brittle diabetes. Residual hepatic lesions identified on surveillance are currently being observed by a multidisciplinary team. This case demonstrates how disease progression changes operative thresholds and reshapes the landscape for subsequent interventions at each decision point in MEN1.ConclusionThis case illustrates the evolving decision-making process in advanced MEN1, where multidisciplinary teams must continuously balance oncologic control against preservation of pancreatic function, metabolic consequences of surgery, and long-term quality of life. Early recognition, individualized surgical timing, and longitudinal multidisciplinary management are paramount.
