    Background:

    • Stenosis of the middle cerebral artery (MCA) can lead to significant neurological deficits.
    • Understanding the natural history and treatment outcomes for MCA stenosis is crucial for patient management.

    Purpose of the Study:

    • To review the clinical course of patients with middle cerebral artery stenosis managed non-surgically.
    • To evaluate the efficacy of medical treatment, including anticoagulation, for MCA stenosis.

    Main Methods:

    • Retrospective review of 16 consecutive patients diagnosed with MCA stenosis between 1970 and 1977.
    • All patients received non-surgical medical management, primarily anticoagulation.

    Main Results:

    • 14 out of 16 patients experienced a benign clinical course with no subsequent transient ischemic attacks or stroke.
    • Two patients developed severe strokes early in the disease course, prior to medical therapy initiation.
    • No specific clinical or radiographic features predicted patient outcomes.

    Conclusions:

    • Non-surgical management, including anticoagulation, appears to yield a benign course in the majority of patients with middle cerebral artery stenosis.
    • The findings support the necessity of randomized controlled trials to assess the efficacy of surgical bypass procedures in this patient population.
