PHILOS plating versus percutaneous K-wire fixation in proximal humerus fracture management

Abstract:
Functional outcomes of PHILOS plating versus percutaneous K-wire fixation for managing proximal humerus fractures (PHFs) are debatable since years. Hence, a prospective observational study of 42 patients revealed that PHILOS plating resulted in significantly earlier radiological union and superior shoulder function scores, though with longer operative times and greater blood loss. K-wire fixation was associated with a shorter surgical duration and minimal blood loss with a higher incidence of complications like malunion and pin tract issues. Thus, we show that PHILOS plating offers a more stable fixation for optimized functional recovery, while K-wire fixation remains a viable, less invasive option for selected cases.
