Nicotine pouches: Harm reduction or initiation pathway? A pharmacy-focused review

Background:
Nicotine pouches (NPs) are a rapidly growing category of tobacco-free oral nicotine products increasingly available in the community, including pharmacies. Whether NPs primarily function as harm-reduction tools for existing tobacco users or as a novel initiation pathway into nicotine and tobacco use, particularly among youth, remains a critical and unresolved clinical and public health question.
Objectives:
To synthesize current evidence on the pharmacology, harm-reduction potential, and initiation risk of NPs; to compare NPs with FDA-approved nicotine replacement therapy (NRT) and combustible cigarettes; and to identify implications and counseling priorities for pharmacy practice.
Methods:
A narrative evidence synthesis was conducted using a structured search of PubMed, Web of Science, and Google Scholar (January 2019-April 2026). Search terms included: "nicotine pouches," "oral nicotine pouches," "tobacco harm reduction," "nicotine initiation," "youth tobacco use," "pharmacokinetics oral nicotine," "pharmacy practice tobacco," and "smoking cessation non-combustible." Thirty-five peer-reviewed articles met the inclusion criteria after screening 214 records. Studies were selected if they evaluated product characteristics, biomarkers of exposure, population-based use patterns, marketing analysis, or clinical outcomes related to NPs.
Results:
NPs deliver nicotine via oral mucosal absorption with slower pharmacokinetics than combustible cigarettes and an abuse liability profile comparable to NRT. Biomarker studies demonstrate reduced toxicant exposure compared to continued smoking; however, complete cessation remains superior. Youth experimentation is rising significantly, driven by flavored formulations, "tobacco-free" marketing, and social media normalization. No randomized controlled trial has demonstrated a statistically significant effect of NP on smoking cessation. Accidental ingestion in young children represents an emerging safety concern.
Conclusion:
NPs occupy a dual position: a potential harm-reduction tool for adult smokers unable or unwilling to quit, and an emerging initiation pathway for nicotine-naïve individuals, especially youth. Pharmacists are uniquely positioned to counsel patients on these distinct risk profiles. Long-term clinical outcome data and evidence specific to pharmacy practice are urgently needed.
