    Summary

    Atrial infarction can cause atrial rupture, a complication often missed, leading to fatal ventricular rupture. Early surgical repair is possible if atrial infarction and its ECG changes are recognized promptly.

    • A significant depression of the PR segment (0.04 to 0.07 mV) was observed.
    • A novel finding was a convexity in the initial portion of the PR segment in leads showing PR depression.

    • Prompt recognition of atrial infarction and its associated ECG findings is crucial.
    • Timely diagnosis may allow for surgical intervention to prevent fatal outcomes.
    • Understanding these specific P wave alterations can improve the diagnosis of atrial infarction.
