Anatomical Leaf Self Expander®: A Novel Approach for Early Maxillary Expansion in Paediatric Patients

Aim:
To describe the clinical management and outcomes of two paediatric patients with severe transverse maxillary deficiency treated with the Anatomical Leaf Self Expander®, a newly designed miniaturised Ni-Ti expander specifically conceived for early interceptive therapy.
Background:
Early maxillary expansion in the deciduous dentition can be challenging when the palatal vault is extremely narrow and conventional expansion screws are oversized for the available space. A compact, self-activating device may facilitate treatment while minimising discomfort and avoiding the need for patient compliance.
Case Report:
Two young patients in the complete deciduous dentition, both presenting posterior crossbite and signs of maxillary constriction, were treated using the Anatomical Leaf Self Expander® bonded to the second deciduous molars. The device provided continuous, calibrated forces without the need for manual activation. Both cases showed complete correction of crossbite, spontaneous alignment of the upper arch, and improvement in facial symmetry and occlusal relationships, with stable outcomes at follow-up.
Conclusion:
The Anatomical Leaf Self Expander® offers a simple and effective approach for early transverse maxillary expansion in children with limited palatal space. Its compact design and self-activating mechanism allow predictable orthopaedic effects with minimal discomfort and no reliance on cooperation, making it a suitable first-line option for early interceptive treatment.
