One-Lung Ventilation in a Pediatric Tracheostomy Patient: A Case Report

Abstract:
A 20-month-old male with congenital lobar emphysema presented for right middle lobectomy. He had previously undergone right upper lobectomy and tracheostomy placement and remained ventilator-dependent. Due to recent accidental decannulation necessitating emergent tracheostomy revision, otolaryngology advised that stoma patency be maintained throughout the perioperative period. As such, orotracheal intubation techniques were not practical options for lung isolation. This case report describes a unique method of achieving one-lung ventilation in a young pediatric patient with a tracheostomy.
