Decoding hidden inceptions: The evolution from indolent to aggressive cancers

Abstract:
Cancer screening detects lesions that meet pathological criteria for malignancy yet may remain clinically silent for years or decades. This paradox exposes a fundamental gap in cancer biology, although the mechanisms driving aggressiveness are well studied, far less is understood about what prevents malignant potential. Here, we propose that tumor indolence is an actively maintained condition of restrained malignant potential, sustained by a multilayer restraint architecture encompassing lineage and epigenetic barriers, epithelial cell competition, immune containment, stromal organization, and vascular and metabolic limitations. Within this framework, progression results not from a single decisive alteration but from the progressive erosion or collapse of these overlapping restraints. Reframing indolence in this way distinguishes biologically restrained lesions from cancers merely detected early and shifts the clinical objective from identifying cancer alone to predicting when restraint will fail, enabling more rational surveillance, risk stratification, and timely intervention while reducing overdiagnosis and overtreatment.
