Sustained Remission Following Limited Dupilumab Exposure in Refractory Eosinophilic Esophagitis

Abstract:
Eosinophilic esophagitis (EoE) is a type 2 immune-mediated esophageal disorder that can be refractory to standard therapy. We present a case of a 49-year-old woman with asthma, chronic sinusitis, and nasal polyposis diagnosed with EoE refractory to proton pump inhibitors, dietary modification, and swallowed topical corticosteroids. She was treated with dupilumab in 2023 but discontinued it after 3 months due to side effects. Subsequent esophagogastroduodenoscopies demonstrated sustained histologic and endoscopic remission up to October 2025 (26 months later), after which recurrence of disease was confirmed. This case highlights 2-year sustained remission following limited exposure to dupilumab in refractory EoE.
