[Diagnostics and treatment of hepatocellular carcinoma and biliary carcinomas-Living guidelines]

The S3 guidelines "Diagnostics and treatment of hepatocellular carcinoma and biliary carcinomas" (version 5.2, June 2025) are conceived as living guidelines and are regularly updated. They represent an evidence-based foundation for the interdisciplinary care and integrate current developments in diagnostics, imaging and treatment.For HCC, treatment is guided by the BCLC stage, liver function, and the patient's general condition. In early-stage disease, liver resection, liver transplantation, and local ablative therapies represent potentially curative treatment options, whereas interventional and modern systemic therapies are used in more advanced stages. For biliary carcinomas, assessment of resectability is of particular importance, as R0 resection represents the only curative treatment option. In advanced disease, modern immunotherapies, molecular characterization, and targeted therapies have become increasingly important.
