A Review Highlighting Emerging Frontiers in Hypertension Management: Integrating Novel Therapeutics and Holistic

Abstract:
Despite the availability of various antihypertensive medications, hypertension (HTN) remains a major global cause of heart disease, stroke, renal damage, and early mortality. This narrative summary describes current advances and new directions in HTN treatment beyond prescription drugs. We used PubMed, Scopus, Web of Science, and Google Scholar to conduct a comprehensive literature search for papers published between 2000 and 2026. A careful evaluation and combination of pertinent clinical trials, systematic reviews, meta-analyses, international recommendations, and important research about innovative blood pressure therapies, device-based interventions, digital health tools, personalized medicine, and advances in public health were conducted. In addition to highlighting novel pharmacological treatments such as aldosterone synthase inhibitors, RNA interference- based medicines, and endothelin pathway regulators, this study examines the shortcomings of traditional blood pressure drugs. Device-based methods, such as renal denervation and baroreceptor activation therapy, along with digital therapeutics, remote healthcare, wearable monitoring equipment, and AI-supported clinical decision-making tools, are also being explored. Additionally, personalized medicine strategies involving genetics, metabolomics, biomarker-directed treatment, lifestyle improvements, and public health programs, including WHO HEARTS and India IHCI, are discussed. While many emerging strategies show promising results in reducing blood pressure, significant constraints persist regarding evidence of long-term cardiovascular outcomes, cost, availability, and applicability across varied populations. Future HTN management will likely require integrated, patient-focused, technology-driven, and precision-guided approaches to achieve lasting and equitable cardiovascular risk reduction worldwide.
