Borderline personality disorder: Challenges in an inpatient setting

Abstract:
ObjectiveBorderline Personality Disorder (BPD) remains one of the most challenging conditions encountered in inpatient psychiatry, particularly in regional settings where access to specialised psychological care is limited. Drawing on the personal experience of a public psychiatric specialist practicing in regional Australia, this piece reflects on diagnostic dilemmas, therapeutic boundaries, and the emergence of other challenges unique to the treatment of BPD. A large part of diagnostic complexity in BPD often arises from overlapping symptomatology of differential diagnoses such as Bipolar disorder, Schizoaffective disorder, Depression, complex Post-traumatic stress disorder and other psychiatric conditions, leading to diagnostic ambiguity and a therapeutic drift towards pharmacological over-intervention. Such practices, while well-intentioned, may inadvertently reinforce maladaptive dependency and prolong recovery.ConclusionThere is significant misdiagnosis with conditions that have symptoms that superficially resemble BPD traits. This subsequently leads to the delivery of inappropriate treatments. It is the author's opinion that this is an unintended fallout of the mental healthcare system's risk averse approach. Unfortunately, this leads to a gross deviation from evidence-based treatments for BPD, resulting in poorer outcomes for patients and placing a significant stress on public mental healthcare in Australia.
