Abstract:
Recurrent asthma exacerbations despite optimal therapy warrant investigation for alternative causes. In this setting, silent aspiration is a key but often forgotten trigger of airway inflammation that should be considered in the differential diagnosis. We present a patient with long-standing severe Type 2 asthma complicated by recurrent exacerbations of an infective nature that was ultimately found to be associated with gastric band slippage causing gastric stenosis and delayed gastric emptying. Exacerbations ceased after surgical intervention and removal of the band. The report highlights the importance of considering gastrointestinal causes, particularly in people with prior gastric surgery, as a reason for poor asthma control and repeat exacerbations. We review literature pertaining to this field and emphasise the need for a multidisciplinary approach (including surgical intervention) to identify and effectively treat silent aspiration.
