Background:
Accountable care organizations operating across multiple electronic medical record (EMR) systems face fragmented quality reporting that limits transparency and shared accountability.
Project Rationale:
Inconsistent measure definitions, nonstandard reporting formats, and resource-intensive reconciliation across EMR platforms prevented reliable performance comparison and delayed identification of care gaps.
Project Summary:
A multidisciplinary team developed and implemented a simplified aggregation framework to extract, normalize, attribute, aggregate, validate, and distribute quality data across multiple EMRs. Three accountable care organization measures were tracked during the 2025 calendar year. Blood pressure control improved from 67% to 70%, depression screening from 55% to 68%, and breast cancer screening from 45% to 50%. An interactive dashboard replaced fragmented reports, shifting quality conversations toward actionable improvement.
